Note to Certifying Officer regarding PERM filing date This ETA 9089 was ready to file prior to 8619, which is the expiration date of the PWD. However, on 8519 8619, we, as attorney for employer, were unable to file PERM due to DOL system disabling PIN associated with attorney subaccount. When clicking submit, system indicated PIN was invalid. We made two more attempts to submit on 8519, but warning message appeared, then indicated that account was disabled that we would need to contact DOL to reinstate PIN. We emailed DOL on 8519 to reinstate PIN. A response from DOL was received on 892019. As confirmed by employer, Standard Insurance Company has had the same account PIN since 2011, this account has been used throughout the yrs to submit PERMs. This has never occurred with this PERM account. We do not know why PERM system indicated PIN was invalid, which subsequently disabled account, resulting in not permitting us to submit this form on 8519 or 8619, interfering with ability to file prior to PWD expiry. Upon request, we can produce evidence.
